Submit Documents

Please Follow this link if you have an Open Document Request

Log in using your assigned username and password. These are the same credentials you used to elect benefits.
Select the Documents tab on the left of the screen.
Select My Documents (the screen will state how many documents are required).
Drag or browse to upload a file into the boxes displayed on the screen.
Click Submit.

2024 Benefit Changes and New Enrollment

If you are a newhire with benefits effective in 2024, or have a Qualifying Life Event which effects 2024 benefits, please update here. Some examples of Qualifying Events are:

Birth of a child or Adoption
Marriage
Loss or Gain of Other Coverage
And more

1. Click The Open Enrollment button to the right.

2. Login with the following:

Username: First name initial + First 6 letters of last name + Last 4 digits of SSN

Password: Date of Birth YYYYMMDD

Example

Name: John Smith, SSN #: 123-45-6789,

Birthdate:May 3, 1974

Username: jsmith6789

Password: 19740503
*You will be prompted to setup a permanent password to use moving forward.
